MEMBER REGISTRATION CHECKLIST FOR 2021

 

  1. Complete the Cairns Physie Registration form
  2. Pay Cairns Physie Membership Fee
  3. Nominate your payment option for Cairns Physie Annual Fee
  4. Pay the BJP Registration Fee (These details will be supplied once you have registered with Cairns Physie.)

CAIRNS PHYSIE REGISTRATION FORM

1. All existing and new members are required to complete an Online Registration Form for Cairns Physie.

  • Please provide a list of any existing medical conditions that we should be aware of e.g. Asthma, Allergies or other. Please list all that apply

    I give permission for images and footage of me to be used by Cairns Physical Culture for publicity purposes. Photography and video footage may be used in class, appear on our website, Facebook page and/or in various marketing materials.
    I understand, acting for myself, agree that participation of Physical Culture classes run by Cairns Physical Culture Club are at my own risk. Any accident or injury incurred at such times is in no way the fault of the associates or committee members of the club and are my own responsibility.

Cairns Physie Membership Fee $40

2. All members are required to make a one-off payment of $40 to Cairns Physie Club. This fee is due by 5th March 2021. If you are a NEW MEMBER, your fees are due 3 weeks after your starting date.

Cairns Physie Annual Fee $540

3. Our annual fee covers all group classes throughout the year for Seniors and Ladies

There are 2 payment options for your annual fee:
Payment option 1:  Early Bird Discount ($486) – 10% discount applies when paid in full before 5th March 2021
Payment option 2:  4 instalments per year ($135) – First payment due by 5th March 2021

Payment Method – Bank transfer
When paying by bank transfer please include your first initial and surname, followed by the code: FEES2021 to help us track your payment. For example: Sharon Jane pays her annual fee, her bank transfer code will be ‘SJANE FEES2021’ If Sharon was paying her 1st of 4 instalments, her transfer code will be SJANE P1

$135 Instalment x 4 payment dates:

Term 1 due by 5th March – 8th February 2021 to 1st April 2021 (8 weeks) – $135

Term 2 due by 7th May – 19th April 2021 to 24th June 2021 (10 weeks) – $135

Term 3 due by 30th July – 12th July 2021 to 16th September 2021 (10 weeks) – $135

Term 4 due by 15th October – 4th October 2021 to 25th November 2021 (8 weeks) – $135

BJP REGISTRATION FEE

4. All members are required to pay a one-off annual fee of $99 directly to BJP Physie by the 12th March 2021. Cairns Physie will provide these details upon registration with our club.
